Uverse Health Privacy Policy

Uverse Health LLC provides a healthcare access, care coordination, and healthcare operating platform connecting patients, healthcare professionals, organizations, and participating healthcare partners. This Privacy Policy explains how Uverse Health collects, uses, stores, protects, and discloses information when you use our websites, applications, services, and related healthcare-platform features.

Information we collect

We may collect information you provide directly, including your name, email address, telephone number, account credentials and authentication data, profile information, organization affiliation, professional credentials, licensing information, care requests, referrals, communications, uploaded records, consent preferences, and other information submitted through Uverse Health.

Depending on your role and use of the platform, information may include healthcare-related information you supply, provider credentialing information, organizational information, payment or transaction records, support requests, and communications with participating users or organizations.

We may also collect device and technical information such as IP address, browser or device type, operating system, application version, diagnostic information, security and authentication events, usage activity, and other information needed to operate, secure, and improve the platform.

How we use information

We use information to:

  • create and manage accounts;
  • authenticate users and protect platform security;
  • provide healthcare access and care-coordination tools;
  • support provider onboarding and credentialing;
  • manage referrals, communications, and service requests;
  • connect users with participating healthcare professionals and organizations;
  • process support, compliance, safety, and account-deletion requests;
  • prevent fraud, misuse, unauthorized access, and security incidents;
  • maintain legal, financial, operational, and audit records;
  • improve platform performance and user experience; and
  • comply with applicable legal and regulatory obligations.

How information may be shared

We may share information with participating healthcare providers, organizations, service partners, contractors, technology providers, payment processors, credential-verification providers, and other parties necessary to operate or support services requested by the user. Service providers may use information only as permitted to provide their services to us and subject to applicable contractual or legal obligations.

We may also disclose information when required by law, regulation, court order, legal process, fraud-prevention needs, security requirements, or legitimate compliance obligations. Uverse Health does not sell personal information.

Healthcare and regulated information

Certain information may be subject to healthcare, professional, financial, contractual, or other regulatory requirements. Where applicable, Uverse Health applies administrative, technical, and organizational safeguards appropriate to the nature of the information and services being provided.

Uverse Health is not a substitute for emergency medical services. If you are experiencing a medical emergency, call 911 or your local emergency number.

Data security

Uverse Health uses administrative and technical safeguards designed to protect information, including encryption in transit, access controls, authentication, logging, monitoring, and role-based authorization. No system can guarantee absolute security, but we work to reduce unauthorized access, misuse, alteration, and disclosure.

Data retention

We retain information only for as long as reasonably necessary to provide services, maintain security and audit records, meet contractual requirements, prevent fraud, resolve disputes, and comply with legal, financial, healthcare, regulatory, or recordkeeping obligations.

Some records may need to be retained after account deletion where required for healthcare, payment, fraud-prevention, audit, contractual, compliance, or legal purposes. Retained records are restricted from ordinary account use and marketing and are deleted or anonymized when the applicable retention period ends.

Account and data deletion

Users may request deletion of their Uverse Health account and associated eligible data through the application or through our public account-deletion page.

Eligible profile information, marketing preferences, device tokens, non-required metadata, unused uploads, optional demographic data, and ordinary application activity may be deleted or anonymized. Certain records may be retained where required by law, healthcare obligations, payment and financial requirements, fraud prevention, security, audit, contractual duties, or legal hold.

User choices and rights

Depending on your location, you may have rights to request access, correction, deletion, restriction, or portability of certain personal information. You may also manage communication preferences, consent settings, and account information through available platform controls. Submit requests to privacy@uversehealth.com.

Children's privacy

Uverse Health is not intended for children to create independent accounts unless expressly authorized through a parent, guardian, healthcare organization, or legally permitted workflow. We do not knowingly collect information from children in violation of applicable law.

International and regional availability

Availability of Uverse Health services and privacy rights may vary by country, region, organization, regulatory requirement, provider participation, and partnership arrangement.

Changes to this policy

We may update this Privacy Policy as our services, legal obligations, and privacy practices evolve. The updated policy will be posted on this page with a revised effective date.
